The Lakeland Cardinals program relies on parent participation to be successful. At registration, parents are required to pay a $50.00 Non-Volunteer fee per family. This fee is refundable after performing 4 hours of league approved volunteer service.
Reimbursement Process:
1. Perform the required volunteer time benefiting the Lakeland Cardinals
(some examples are listed below).
2. Obtain a volunteer verification form signed by your childs head coach and complete the form. Make sure you check one of the boxes or describe your volunteer activity in the space provided (ONLY CURRENT YEAR FORMS WILL BE HONORED).
3. Make a copy of the form for your records, and then return it to your head coach for submission. Reimbursement will not be given if the form is not completed and submitted properly.
4. Reimbursement of $50.00 will be mailed to the volunteer after the treasurer has received the properly completed form.
6. Volunteer forms not received or postmarked prior to December 1st of the current year will not be honored for reimbursement.
